- Identify the federal program affected by the requested waiver;
- Describe how the waiver will increase the quality of instruction for students and improve the academic achievement of students;
- Describe specific measurable educational goals, in accordance with the current law, for the state and each district that would be affected;
- Describe the methods to be used to measure annual progress;
- Explain how the waiver will assist the state and each district to reach those goals; and
- Describe how schools will continue to provide assistance to the same populations served by the programs for which the waiver is requested.
